Halton Health Improvement Live Well Team – Your Health, Your Future
About the Live Well Team
The Live Well Team, part of Halton Borough Council’s Health Improvement Team, works to reduce health inequalities across Halton by delivering targeted, preventative health programmes. Their work is shaped by the Halton Joint Strategic Needs Assessment (JSNA), which highlights significant health challenges in the borough, including high rates of cardiovascular disease, cancer, and premature mortality.
The team’s approach focuses on reaching those least likely to access traditional health services, using community outreach and data-driven targeting to deliver impactful interventions.
The Project: Your Health, Your Future
With support from Halton Housing’s 20 for 20 campaign, the Live Well Team launched Your Health, Your Future—a targeted NHS Health Check initiative aimed at Halton Housing’s most vulnerable residents.
Over a three-month period, the project will deliver Health Checks in accessible community venues, including Halton Housing neighbourhoods. A qualified practitioner will work alongside Halton Housing’s community workers to identify and engage residents aged 40–74 who are most at risk but least likely to attend a GP surgery. A dedicated booking system will ensure flexibility and convenience for participants.
Community Impact
The project aims to reach around 150 residents, focusing on those living in Halton’s most deprived areas. By delivering Health Checks in familiar, local settings, the initiative removes barriers such as transport, cost, and awareness—supporting early identification of health risks and connecting residents to wider Live Well services like smoking cessation, weight management, and mental health support.
This pilot will also generate valuable insights into uptake and engagement, helping shape future delivery and scale-up opportunities.
Working Together with Halton Housing
This project is a true partnership between the Live Well Team and Halton Housing. While the Health Improvement Team provides clinical expertise and resources, Halton Housing’s community workers play a vital role in identifying residents, promoting the service, and providing accessible venues. Together, they aim to build trust, reduce barriers, and improve health outcomes for Halton Housing customers.
